The sub-title states: Development of a comprehensive knowledge-based framework to address the needs of Canadians with long-term disabilities who are aging. The banner contains 4 images: First a View of a visitable house with well-integrated sloping pathway from the street to the main entrance. A senior using a wheelchair is approaching his friend’s house.  This represents the housing component of project. Secondly, a senior is transferring out of his car into a wheelchair while a friend shields him with an umbrella from the rain. This represents the caregiving/home supports component of project. Next a senior walking his two dogs in a park like setting. This represents the community participation component of project. Finally, a visually impaired senior is waiting for public transportation with a city bus approaching behind her. This represents the transportation component of project.

December 2008- Livable and Inclusive Community Model Development
Based on a review of the literature, an examination of other initiatives focused on seniors or persons with disabilities, and previous research conducted by CCDS, it was determined by the Project Team that livable and inclusive communities consist of ten common elements guided by six principles to determine the degree to which the element reflects livable and inclusive communities and understanding the interconnections between them.

January 2009- Training Sessions- Community Evaluation Tool
The six pilot communities received on-site training on a community self-assessment evaluation tool developed by the Canadian Centre on Disability Studies.  The evaluation tool contains the livable and inclusive communities for seniors with disabilities model, description of the process and steps taken by the Working Group to gather, consolidate, and review the data.  Indicators have been developed for each element to measure the extent to which the element is present in the community being evaluated.  By gathering data for the tool’s indicators, the pilot communities have been able to measure the extent to which their communities are livable and inclusive for seniors with disabilities. 

February 2009- Community Planning Tool
The central purpose of the Community Planning Tool is to lead the pilot communities through sequential steps towards identifying the short and long term priority areas as it relates to seniors with disabilities; linking the priority area with livable and inclusive community principle(s); identifying the actions to be taken; and identifying who should be involved, the associated costs, and anticipated time frame for completion of action. 

March 2009- Think Tank March 26-28 in Winnipeg, Manitoba
The Think Tank will be an opportunity for representatives from the six communities to meet face to face, to learn from each other, and build a consensus for issues at the Federal policy level. Recommendations will be developed based on the information collected and lived experience of seniors with disabilities to take forward by the Project Team.

April 2009- Final Report
The CCDS Project Team will synthesize the information collected by the communities, priorities addressed and future actions to be taken on the local and national levels in the Final Report. The report will be widely distributed, including the Regional Team Leaders and Community Working Groups.
